Limitations and Risks of Telehealth

Because of these limitations, APNS providers may determine that your medical concern cannot be safely managed through telehealth. If this occurs, you may be instructed to schedule an in-person evaluation with your primary care provider, visit an urgent care center, seek emergency treatment, or obtain evaluation from an appropriate medical specialist.

Provider Clinical Judgment

All medical decisions made by APNS providers are based upon independent professional judgment, current standards of medical practice, applicable laws and regulations, and the information available during your evaluation.

Providers reserve the right to decline, discontinue, or defer treatment whenever they determine that:

  • Additional evaluation is medically necessary;
  • Telehealth is not appropriate for your condition;
  • A requested medication or treatment is not medically indicated or cannot be safely prescribed;
  • State or federal laws prohibit the requested service; or
  • Providing care would otherwise be medically, ethically, or legally inappropriate.

Payment for a medical evaluation does not guarantee that a prescription, diagnosis, laboratory order, imaging study, referral, or requested treatment will be provided.

Alternatives to Telehealth

Participation in telehealth is voluntary.

Patients may choose to receive healthcare through other available options, including:

  • Their primary care provider;
  • An urgent care center;
  • An emergency department when appropriate;
  • An in-person visit with APNS, if available; or
  • Another qualified healthcare professional.

Patients are encouraged to select the care setting that is most appropriate for their medical needs.

Consent to Telehealth

By participating in a telehealth appointment with APNS, you acknowledge and agree that:

  • You understand the benefits and limitations of telehealth.
  • You have had the opportunity to ask questions regarding telehealth services.
  • You voluntarily consent to receive healthcare through telehealth.
  • You understand that you may withdraw your consent and discontinue the telehealth visit at any time before treatment is completed.
  • You understand that a provider may terminate or convert the visit to an in-person referral if telehealth is not appropriate for your medical condition.

Patient Intake and Health Information

Information you provide through APNS registration forms, questionnaires, patient intake documents, and telehealth visits is used to assist our providers in evaluating your health and delivering appropriate medical care.

You are responsible for ensuring that the information you provide is complete, accurate, and current to the best of your knowledge. Inaccurate or incomplete information—including omitted medical conditions, medications, allergies, or symptoms—may affect the provider’s ability to safely evaluate or treat you.

Your health information is maintained in accordance with applicable federal and state privacy laws, including the Health Insurance Portability and Accountability Act (HIPAA), and is used, disclosed, and protected as described in APNS’s Notice of Privacy Practices.

Communications

By registering for services with APNS, you consent to receive communications related to your healthcare, including appointment reminders, registration information, secure portal messages, prescription notifications, laboratory updates, billing communications, and other healthcare-related notifications by email, text message, telephone, patient portal, or other electronic means, as permitted by law.
